If you’re a first-time patient, we will send you the paperwork through the US mail or email, or you can go to the forms tab to download the papers for printing. Making an appointment
Please call the clinic office during normal office hours. It is necessary to call ahead to schedule an appointment. Every effort will be made to provide you with prompt attention at the time of your appointment. Due to unscheduled nature of emergencies occasional delays may occur. Please be sure to supply our office with an emergency contact number in case an emergency should arise. Our office will notify you if possible in advance when Dr. Melissa Kinder will be delayed. If you are unable to keep your appointment, we request you notify our office 24 hours in advance. |
Insurance Information
Be sure to bring your Insurance Identification Card and any other necessary information. We recommend you call your insurance to ensure Dr. Kinder is a preferred provider. It is your responsibility to submit the deductible or co-pay amount at the time of your appointment. It is also your responsibility to provide a referral form if necessary for your insurance company. We accept Medicare, but you will be responsible for any charges Medicare does not pay. |
